What challenge were they looking for help with?

The provider commissioned an independent, external review of how its safety arrangements were working across all of its roped activities.

The brief focused on:

• assessment of risk
• selection, inspection and maintenance of activity equipment
• internal training and assessment processes for instructors, rescuers and trainers
• how staff competence and currency were maintained
• how staff monitoring supports consistent practice and assurance

Like many organisations, the provider wanted reassurance that established systems were continuing to achieve their intended outcomes across a complex operational environment.

The approach we took

Adventure RMS undertook a structured review with practical observation of delivery at the core of the process.

The review included

• Visits to a range of centres
• Observation of activity delivery
• Observation of staff training and assessment processes
• Discussions with instructors, trainers and managers
• Review of supporting documentation and operational procedures.

Two consultants, supported by Adventure RMS consulting inspectors, worked collaboratively throughout the project, allowing observations, ideas and emerging findings to be shared and challenged as the review progressed.

A key objective was to understand how the organisation’s documented procedures were interpreted and applied in day-to-day operations across different centres.

Talking to the team

Conversations with staff were an important part of the review. They helped identify how procedures were understood, where local practice had developed, where opportunities existed to improve consistency across locations, and where systems were creating clarity or confusion. Observations also helped establish whether control measures were realistic, understood, and consistently applied, as well as how day-to-day delivery was taking place.

The document review was used to compare written arrangements with operational reality, asking whether the written system supported staff to deliver safely and managers to maintain effective oversight.

The findings were compiled into a final report with associated recommendations and presented to key staff within the organisation. Providing an opportunity to discuss the findings and recommendations, explore the rationale behind them, and give any further clarity and context.

Why use an independent reviewer?

An external review can provide a different perspective to that available internally.

Those working within an organisation often become highly familiar with existing arrangements. This can make it difficult to identify areas where systems have grown unnecessarily complex, where practices have gradually diverged across locations, or where operational reality is no longer supported by documentation.

Independent consultants bring:

• Experience from across multiple providers and sectors
• Insight into emerging good practice
• The ability to test assumptions and challenge established thinking

The aim is not to criticise existing arrangements but to identify opportunities for improvement and support organisations in making informed decisions.

The review helped identify where arrangements could be simplified, expectations clarified and existing good practice shared more effectively across teams and centres. These changes, in turn, should help make the system more reliable across multiple sites and teams.

The common thread across outcomes was the benefits of making the safety management system easier to apply well and consistently. In this case, the review identified ways to turn complexity into clearer priorities: consistent practice, practical assurance and safety systems that support the people using them.
